Self Refer for Physio
-You are over the age of 17+
-You are registered with a Vale of York CCG including Pocklington
-You have a musculoskeletal pain problem (self referral isn't available for mobility rehabilitation, neurological problems, or continence rehabilitation)
If you have a personal history of cancer please do not use the self referral form, you should see your GP in the first instance

Self Referral for Podiatry
Harrogate and District foundation trust provide Podiatry services for North Yorkshire and offer a range of services, including general core podiatry, nail surgery, management of high risk feet and muscular and skeletal imbalances and injuries and are now able to accept direct self-referrals from patients.
Who can we treat?
People with a foot need for NHS Podiatry:
- Foot wounds
- Foot infections that have needed antibiotics.
- In growing toenails that are causing pain or infection
- Painful corns and hard skin
- Foot pain that is affecting someone’s daily life or quality of life.
- A foot deformity (i.e., bunion) affecting someone’s daily life or quality of life.
- Pain when walking that is affecting someone’s daily life or quality of life.
People with a medical need for NHS podiatry care:
- People with diabetes who have been assessed as at moderate or high risk of developing foot complications at their annual assessment at their GP surgery.
- People with long term health conditions that put their feet at increased risk of foot complications such as: Rheumatoid Arthritis, Peripheral Vascular disease (including Raynaud’s syndrome), people with neurological dysfunction affecting their feet, people with connective tissue disorders.
- If you are not sure if you have a medical need, please write all your medical conditions and concerns on the referral form and this will be assessed by a Podiatrist.
Who can’t we treat?
- Cut toenails for people who do not have a medical need for NHS Podiatry care.
- Treat people who have Diabetes but are at low risk of developing foot complications.
- Treat Verrucae’s or warts
- Treat small amounts of hard skin that are not causing pain or discomfort.
- Treat fungal infections of the skin (i.e., athletes' foot)
- Treat foot deformity (i.e., bunions or flat feet) that are not causing pain

Talking Therapies - Self Refer
Find an NHS talking therapies service
If you live in England and are aged 18 or over, you can access NHS talking therapies services for anxiety and depression.
A GP can refer you, or you can refer yourself directly without a referral.
NHS talking therapies services offer:
- talking therapies, such as cognitive behavioural therapy (CBT), counselling, other therapies, and guided self-help
- help for common mental health problems, like anxiety and depression
- See problems that talking therapies services can treat
You need to be registered with a GP to get talking therapies on the NHS.

NHS e-Referral Tracking
If your doctor has referred you to a specialist for further care you will be asked to book your appointment using the ‘NHS e-Referral’ system.
You will receive a letter from us within 2 weeks giving details of how to book your appointment and any passwords you may need.
To track your e-Referral you will need the following details to log into the e-Referral website:
- Your date of birth
- Reference number – this will be on the first page of your letter
- Password – this will be on the second page of your letter
